Mikisew Cree First Nation Health Department  is seeking a dedicated and  experienced LPN to join our team to become part of a progressive organization with a mandate of improving patient outcomes.  The position of Community Support Nurse is part of a progressive organization with the mandate to improve patient outcomes

  • Often The first point of contact for intake, the successful applicant must display a caring, empathic,  and nonjudgmental attitude toward client(s) and their families .

This position is a part time position and may develop into full time as case load grows.

Roles & Responsibilities

  • Assess, plan, implement, and evaluate individualized care plans , incorporating  evidence-based teaching /education and facilitating referrals pathways to external agencies,  to meet the physical, social, spiritual, and psychological needs of patients struggling with addiction and substance use, as well as additional complex healthcare needs – under the direction of the Health Director
  • Collaborate with an interdisciplinary team to provide compassionate and holistic care
  • Utilize nursing knowledge and skills while implementing basic preventative therapeutic and rehabilitative nursing care.
  • Demonstrate Ability to manage behaviorally complex individuals.
  • Perform delegated independent nursing functions within an interdisciplinary framework adhering to established organizational procedures, policies, guidelines, and regulatory standards within the licensing body scope of practice
  • Participate in data collection and analysis activities Data collection Maintain program statistics and documentation and participate in ongoing program evaluation and reporting.
  • Maintain and coordinate communication between client, staff and physician /Practitioner .
  • Work collaboratively with external health agencies ( such as but not limited to VODP and Nunee Health)Address clients and family members with concerns with empathy while respecting confidentiality and privacy.
  • Must demonstrate effective communication skills by conveying necessary information accurately, listening effectively and asking questions when clarification is needed.
  • Give timely shift change report to co-workers/care team members consisting of pertinent and relevant information for assigned clients.
  • Participate in scheduled team meetings ( in person or virtually) , providing necessary information/input and solution-oriented suggestions
  • Attend mandatory staff development activities training programs. Participate in continuing education activities to ensure competency as per regulatory requirements.
  • Participate in organizational committees as necessary.
  • Assist in the training and mentorship of new Team Members.
  • Record and report the need for maintenance or repair or equipment and remove any faulty equipment from use.
  • Follow proper reporting procedures for actual or potential accidents and/or incidents to ensure follow-up and prevention.
  • Demonstrate knowledge and understanding of health policies and procedures and ability to reference them from appropriate manuals.
  • Comply with the Organization’s Infection control policies and procedures including Infection Prevention and Control to ensure a safer working environment for self and others.
  • Ensure on-going support to clients through the development of mutually agreed upon contact plans, maintaining regular appointments and telephone calls.
  • Demonstrate fitness for work.
  • Consistently demonstrate the ability to take the initiative to make decisions/choices without direct supervision.
  • Cleaning and administrative duties as assigned.
  • Other tasks as assigned within the scope of practice.

Qualifications

  • Graduate of an accredited school of Nursing.
  •  Current practicing registration as an LPN within Alberta.
  • Able to work efficiently as part of a team as well as independently.
  • Ability to communicate effectively verbally and written.
  • Computer literacy, including effective working skills of MS Word, Excel, e-mail, and virtual conferencing essential.
  • Provision of clear Criminal record check and vulnerable person sector  prior to assuming the position.
  • Minimum 2-year nursing experience preferably in an acute care setting.
  • Experience in addictions in an asset.
  • Travel required from Ft McMurray to Ft Chip – usually on smaller aircraft.
  • Intermittent physical activity including walking, standing, sitting, lifting and supporting participants.
  • Possess Superior time management skills, multitasking skills, and the ability to prioritize tasks with minimal supervision and complete assignments  within established time frames.
  • High level of sound and independent judgment, reasoning and discretion.
  • Public speaking and presentation experience is an asset.
  • High ethical standards and professionalism, with a demonstrated ability to adhere to strict confidentiality practices.

 Other Requirements

  • Excellent analytical skills and strong attention to detail.
  • Part-time hours:  24- 32 hours per week  (Tuesday- Thursday). Schedule may require modification as per client needs and employer discretion
  • Overtime /on-call may be required on occasion
  • Valid class five (5) Driver’s License.
  • Willingness to submit to an oath of confidentiality.
  • Successful Completion of Criminal Record Check.
  • Willing to work flexible schedule.
  • This position will work various hours to meet the needs of business operations. May include days, evenings, weekends, and holidays.
  • Willingness to travel
